Mining critical findings report checklist
Document risk conditions with photo evidence at the moment and place. Raised in the field, with or without signal, and traceable with photo, GPS and time.
On every mine site there are conditions a worker sees and knows are wrong: a loose section of ground support mesh, an eroded berm, a badly applied lockout point. The problem is not spotting them — it is that the critical findings report ends up in a notebook nobody reads, with no photo, no location and nobody accountable for closing it.
A properly raised finding is a fatality avoided. And the evidence matters twice over: when the risk materialises, the difference between an investigation and a regulatory penalty usually comes down to proving that the condition was reported, to whom, when, and what was done about it.
