Confined spaces are the deadliest setting in the oil and gas industry: storage tanks, separators, columns, pipelines and vessels that held hydrocarbons or gases. The danger is twofold and invisible: an atmosphere deficient in oxygen or laden with H2S can kill in seconds, and a large share of the victims are those who go in to rescue without equipment. The statistics are brutal: improvised rescuers account for a huge fraction of the fatalities.
The entry permit exists so nobody goes in without the atmosphere having been tested, energies isolated, the space ventilated and an attendant and a real rescue plan in place. In the field it is easy for the tank not to have been purged properly, for the test to have been taken only at the opening and not at the bottom, or for rescue to be left “to sort out at the time”. This checklist forces every one of those points to be closed before authorising.
