A near miss is the free warning: an event that could have ended in a leak, a fire, an explosion or a fatality, but which by circumstance or luck harmed nobody. In oil and gas, where the potential consequences are catastrophic, every near miss reported is invaluable information about a barrier that failed. The real problem is that most go unreported: fear of sanction, “nothing happened”, or simply no easy way to record it at the time.
When the report is lost, so is the chance to fix things before the same condition produces the real accident. A good near-miss report captures what happened without hunting for culprits, assesses what the worst outcome could have been, analyses why the control failed and defines concrete actions with an owner and a deadline. A reporting culture is built by making it easy and blame-free.
