Mining equipment pre-operational checklist
Heavy machinery check before every shift. The operator completes it in the field, with or without signal, recording the condition of the equipment.
A 200-tonne haul truck with a degraded service brake does not forgive. Nor does a loader that goes out to work with a hydraulic leak nobody looked at. The pre-operational check is the last barrier before equipment goes into production, which is why it has to be done by the operator who will drive it, not signed off as a formality.
The real problem is not that checklists are missing: it is that they are filled in on paper, without photos, and the faults found get lost between the tool store and the workshop. When the retarder fails on a ramp, it is too late to go back and see whether anyone had reported it.
