Incident report checklist
Recording of faults, emergencies and problems with follow-up. Completed in the field as soon as the incident happens — with or without signal.
In a facility, incidents give no warning: a lift that stops with people inside, a leak flooding a basement, a power cut, the start of a fire. What determines whether the problem is contained or escalates is how quickly it is reported and the response triggered — and what later allows you to learn from it is that it is properly documented.
The common mistake is treating the report as an afterthought, written from memory hours later. By then the exact time, the photos, the witnesses and the real conditions are gone. And without that record, management is exposed to residents or the client, and to the insurer, which demands timely evidence before covering the damage.
How to use it with CaptaLog
Copy this template and adapt it to the incident types in your operation. Whoever spots the problem reports it from their phone at the time, even with no signal, with photo, location and exact time. The incident is classified by criticality and becomes an action with an owner and a date, to follow up through to close-out with root cause.
